She looks awful with those horrid cheek implants. Sorry, Madonna, but you just shouldn't have done that to yourself.
Is she anorexic? Look at her hands. She is too thin.

I've never liked the cheek filler/implants, but I think they're not obvious or distracting in any of the other photos from that night...I think catching her mid-sentence with her mouth kinda puckered did her in.
I saw the same picture somewhere else, and the cheeks didn't look so pronounced. Looks like the site where you got the pic from did a bit of photoshop editing to enhance here cheeks. Other than the cheeks, she looks good.
Sadly, it looks like Mo's gotten bitten by the body dismorphic syndrome that MJ & Pete Burns have. Hopefully, she'll stop before her face explodes (like Pete's lips did) or cave in (like MJ's nose seems to be headed).
I'm all for a nip/tuck (God knows my crowsfeet & nordic nostrils could use a fix) but she didn't need it to begin with.
