I come from a time when everyone was Mr. Mrs. or Miss unless you really knew them well or they tell you it is okay to call them by there first name. In my younger days I worked for many old Hollywood people. After 20 years of working for one she said to me call me Jean instead of Mrs Schaffner, I felt like Mary Tyler Moore when Mr. Grant said, "call me Lou", we both could not do it.
I also can't stand when someone is called by there last name especially in a newspaper, magazine or internet story.
By the way, I still call my aunts and uncle aunt and uncle whatever. I couldn't think of any other way to address them.
I sure do miss when manners came first.
To the man and his doctor, when he calls you by your first name simply correct him. I assure you it won't happen again.
I address all my patients by their surnames, with appropriate titles before. If I know someone is entitled to an identifier other than "Mr," "Mrs," or "Miss," I use it. Even when invited to use first names, I don't, for exactly the reason suggested - I don't want my patients to address me by my first name.
