Facing the Truvada

If the continuing mixed news that HIV diagnoses are down EXCEPT for young gay males doesn't make the community embrace PrEP, then I don't know what will. Read HERE.

Truvada and PreP are not solutions; they are, at best, bandaids. There is no clear-cut black-and-white answer as both PreP and nothing have important pros and cons.

Certainly, the goal is reducing HIV infections. Directly or indirectly, Truvada costs $1000/month/patient. At, say 2% of adult males, there would be over 3 million on Truvada/Prep, running $3 billion/month. A much less costly approach, of course, are using condoms. Of course, the biggest problem with condoms is getting guys, particularly younger guys, to use them EVERY time. BUT, just one month's of Truvada, or $3 billion, could go an amazingly long way in education and outreach.

As someone who has been HIV+ my entire adult life -- 35 +/- years -- I would much rather see $3 billion/month spent on further improving treatments and a cure. To spend that much because 7000/year idiots won't practice safe sex is as crazy as the idiots who make up the 7000. Think about it this way: $36 billion/year divided by 7000 infection/year = $5.1 million per person to stop HIV infection among young gay men. $5.1 million!!!

Kenneth, you are 10-15 years younger than I, but from your blog, I know you have lived through the phases of major epidemic (in the 1980s, over 100 gay men died each and every week from AIDS) to treatable to manageable. There is much much more work and research before there will be an end. But I can't help but think it would be a hugely better return on investment to put the money proposed for PreP into ending the disease sooner. The 7000/year idiots are always going to be idiots; and PreP is not going to change that.

This is an important and relevant topic and the more airing of opinions the better. I am not saying I am right; I am expressing my thoughts. Please continue re: this story -- ALL POINTS AND OPINIONS contribute to better results. Anyway, my 2 cents. Thanks.
