one of the triplets died during childbirth in the pilot episode.
...the remaining triplets and adopted son were raised as triplets...
long story short, the lady was pregnant with triplets and they lost the third baby in delivery. They ended up adopting another baby the same day and raised them all together as triplets (a lot of the other questions you might have with that are pretty well explained in the show, really quite a good program)
I think they should have put triplets in scare quotes and all would have been fine!
